Modified Issues List

As per the discussion at the 5/18/05 telecom, I have added the a Status
and a Proposed Disposition column to the issues list.  The headings now
read (No line breaks in the Excel sheet)

Lines	 Original Text	Suggested text	Notes	
Proposed Disposition	Disposition	Disposed in

I'm not sure we need Section and Lines since these should be in the
description, if applicable.  I'll leave them there for now.

The status column will have the following values.

Status  
- Closed: Action on issue taken and accepted
- Open: No action taken on issue
- Pending Review: Agreed action taken by editor &
    waiting review by TC
- Pending: Waiting for agreed action to be taken
- Withdrawn: Issue withdrawn by issuer
- Duplicate: Issue same as another issue (Example Dup of 07-1)
