as agreed on call, I've now submitted a revised and updated xls table to
KAVI for handling the issues tracking:

Members of the TC can take a look too in order to familiarise themselves
with the proposed process and information required when submitting an issue.
I have checked the validation fields against the v9 document as published to
the list (the drop down list of sections, for example, and the version
numbering)
