Issue 523 (541-557) re-parsed and realigned comments

I went the original email and tried to make sense of it.  Turns out to be 6 complex comments with references back to the RM, but the initial references use CD numbering and the later ones use PR numbering.  Also, there appears to be a point where something got lost.

In the attached, I applied some formatting and then copied in Duane's comments and mine.  It should be clear (as much as I can make out) what are the comments, what are the responses, and where we have issues to discuss.

Go for it.
