Issue list against SOA-RA 0.2

I did a complete read of the document and generates an annoying number of issues.  Many are editorial, so I hope these can be dealt with quickly.  A few are conceptual and I expect will take more time.  An overriding one that I make against the whole document (and I pick on section 4.5 because that is when the nagging discomfort finally coalesced) is that we have lots of great material discussing processes and background material but not enough saying this is what you put in the architecture to deal with it.  In many case, I don't think this will be difficult to do, it just needs to be done.

Anyway, this was a pain to transcribe from marked up hardcopy, so if something doesn't make sense, get back to me and I'll try to reconstruct my chicken scratch.
