Hi Peter,

please find attached my updated version of the spec, along with the 
individual schemas for the codes. I have changed the appendix 
identification scheme to use alphabetic characters for the designators 
which is the standard used by most specifications. I have also updated 
the links etc. accordingly. I have changed the title of Appendix B 
(formerly known as Appendix 2) to 'Code Lists' and corrected and updated 
the http links to the relevant schemas as required.
