Here are my comments on the whole thing:

Section 2.5.1: I think it is unnecessary to restrict code lists to have two or
more members, and that this section can be deleted completely.  For example, a
list of currencies accepted by a Web site may only have one currency initially,
but using a code list allows room for future expansion.  The list of credit
cards that are accepted by that Web site might be empty initially, but again,
there is room for expansion.  So even an empty list can be a perfectly valid
list.

Section 3.6.4: I see no reason why examples of code lists need be machine
readable.  It's a different question to whether the *models* of the code lists
should be machine readable.  What this document lacks, though, is anything
about the XML data model format.  Is it preferred not to include that in this
document?  Will it be included in another document.

Those are my only comments.
