[ubl-lcsc] issues list from instance and fpsc work so far

Hi folks

I'm sending out, as requested, an ongoing issues list as it appears at present, compiled from any 'bugs' that appear during instance and form spec production. It is constantly growing as work progresses and most of it as it stands has already led to some alterations as authorised in 0.81 (were now up to draft 3).
