[ubl-lcsc] [code] Representation Term Schema

The Representaion Terms Schema has introduced a peculiar problem to
sample instance generation. In seeking to add the supplementary component
metadata for codes, it appears there is so much restriction imposed on the
core components schema supplementary component attributes that there
is little or no scope for adding this data to an instance. Is this intentional?

For example there is now only room for version number in an rt:Amount so
we cannot refer to the agency or qualifier.
