Revised NDR XML and PDF files

I am attaching the revised NDR PDF and XML files.  I made the changes
that we talked about in yesterday's conference calls.  I added unique
id's to tables, examples and paragraphs (just in case the need arises
to link at a paragraph level.

I also cleaned up the tagging in the front matter to match the example
that Ken has in his examples.  The only outstanding issue is the list
of contributor's are not being output. I will check with Ken to see if
we can resolve this problem.
