Where:  Page 20, 4.3.1

It would appear, but is not explicitly stated, that the message in both
directions between Service and coordinator are sent with the context in a
header. This seems unnecessary, as the only field that is really needed is the
context-identifier.

For client:coordinator it is slightly less clear if they are meant to be
contextualized, but the same considerations apply.

Various sub-issues arise, whichever way this goes.
