a quick note on faults

Whilst going through the latest WS-Context document to make the changes 
to incorporate the new fault model, I have removed the generalFault 
message. This is for a couple of reasons; a) it wasn't being used 
consistently (the text and the WSDL had strayed apart), and b) it 
doesn't really make sense to have something that means "something bad 
happened, but I can't tell you what" when all other faults are very 
specific. I don't believe we lose anything by removing this, but wanted 
to flag this to the TC in case someone thinks otherwise.
