Title: RST message body template is missing <wst:SecondaryParameters> element.

Description:

The cd-01 version of WS-Trust is missing <wst:SecondaryParameters> element inside the RST message body skeleton on lines 394 - 398 even though the element itself is defined below on lines 419 - 426.

Proposal:

Add a <wst:SecondaryParameters>...</wst:SecondaryParameters> into the RST message body skeleton after line 396.
