Issue 137: fixed by explicitly including and describing 
 the optional Nonce and Created elements, both in sections
 2.1.1.1 and 2.1.3.1
Issue 142: took an initial attempt to address this issue by
 indicating WS-SecureConversation is used but not actually
 required in the example messages that meet the policy
 requirements.
Issue 143: fixed the typo in section 2.1.1.3 where the 
 Nonce and Created elements were in the wrong place.
