Simplied Receive State chart

attached is the Receive message state chart, based on today's discussions.

 Of note:

- Duplicates of delivered messages are still acked.
- Duplicates of  Held messages are dropped without reply.
- Expiry does cause return of a fault


The return of an ack along with the ack for a future message no longer 
shown at this level of state chart.  However this can be allowd as an 
optimization employed by implementations) (see new attached sequence 
diagram for ReturnNotAvailable).
