The document looks great.
 
My two cents:
6.2 - I think uniqueness is the only thing. Information from the consumer is not necessary (random prefixes can be used), but may be useful to ensure reasonably-sized prefixes (not GUIDs... :-() and also to avoid even the statistically negligible chance for a conflict.
6.3.1 - I think it's indeed the same problem.
6.3.2 - If the portlets know about each other using some mechanism (which is defined elsewhere, or out-of-band communication), this shouldn't be an issue.
