Issues 133 & 134

A side-effect of issue 133 still needs addressing:

  Lines 720-721 (17/merged), the following sentence should be
  removed:
   "Any value specified for binary security tokens, or any XML token
    element QName can be specified here."

  Such values are not valid for the KeyIdentifier ValueType
  attribute. The paragraph already identifies the correct values:
   "Each token profile specifies the KeyIdentifier types that may be
    used to refer to tokens of that type."

Issue 134 is closed.

Example 11, line 1303 (17/merged), X509v3 needs to be
X509SubjectKeyIdentifier.
