[xacml] comments on functions v13

1. I notice function:decimal-mod is not included.  Was this
   intentional?  I personally do not think it is necessary, but
   it was originally specified.
2. Typo: dateTime-greater-than-or-equal is specified twice.  The
   second time, the heading should be
   "dateTime-less-than-or-equal" (the text below is OK)
3. Are the dateTime-add-*Duration and dateTime-subtract-*Duration
   functions mandatory?  I believe they are, but just want to be
   sure.
