Made PolicyId and PolicyIdRef type xs:anyURI.  Made RuleId and RuleIdRef type xs:string.
Removed obligations from rules.
Changed references to v1.0 attributes in examples to references to v2.0 attributes.
Removed Section 7.2 on base policy.  Put corresponding material in Section 7.13.
Introduced recommendation to use UTC time in Section A.2.
Included description of the three PEP types in Section 7.1.
