correction to wd-12 examples, itemized

Erik, here are corrections to examples in core wd-12.

Replace "urn:oasis:names:tc:xacml:3.0:schema:wd-12" with
"urn:oasis:names:tc:xacml:3.0:core:schema:wd-12" globally.

Here are the line number corrections.  You should be able to make most
of these changes without messing up the cross-references.

1062/63: Move 'IncludeInResult="false"' from 1062 to 1063
1072/73: Move 'IncludeInResult="false"' from 1072 to 1073
1081: Move 'IncludeInResult="false"' to child Attribute element
