FYI,

This is a recommendation that we should roll into the next edition of the
specification if we make changes. It's not a technical change, just a
clarification in the wording. If we don't re-edit the spec, it should
probably be in the FAQ.

BTW, the schema enforces uniqueness.

The specification should probably explicitly mention that by changing 
the text to 'within the file element' or something.
