General Discussion of the Profiles Duane discussed the proposed changes to version 1 of the profiles. He suggested updating attributes based on experience with use on the Nationwide Health Information Network (NwHIN). This would include additional attributes in the SAML profile. David suggested the committee contact a member of the NwHIN specification factory, possibly Richard Frank. Duane did not know if the NwHIN specification Factory was still meeting. Duane will look into contacting tem to encourage participation. 4. Cross-Enterprise Privacy and Security Authorization (XSPA) Profile of SAML v2.0 For Healthcare Version 2.0 Draft The TC reviewed the XSPA Profile of SAML. The committee requested the draft use the new titles for the examples of use in the document. There was discussion on the use of Organization, Child-Organization, and Facility. More detail on the distinction was requested from the author. In general, an HIE might be an “Organization”, an HIE member might be a “Child-Organization,” and a clinic of that HIE member might be a “Facility.” In Table 3, “Conformance Attributes,” changes were suggested for mandatory vs. optional attributes. The TC suggested use of multi-value attributes where useful. Mike Dufel agreed. David asked what impact the proposed changes might have on the current use of the profile within the NwHIN. Duane took this question as an action item. In Table 1, “Values for Purpose of Use,” Duane has proposed the addition of the purpose of use (POU) COVERAGE to support the Social Security Administration (SSA) request for determining if someone may receive a benefit for a healthcare claim (which must be validated by examining their healthcare record sent by their provider). During the discussion, Duane suggested the profile focus on providing the “container” for values but allow health-specific standard development organizations (SDOs) to specify the vocabulary. Mike Davis agreed and proposed a preference of URI over the less flexible use of OIDs. Based on the discussion above, removal of the POU table 1 was suggested. This will be reviewed at the next meeting. Changes to the Resource Type were discussed. The resource type should support the “ServiceType,” e.g. patient discovery, “ResourceID, e.g. patient, and ResourceType, e.g. Document. The committee discussed what data type are we requesting. There may be a more general way of specifying data type than pointing to the NwHIN specification. Duane mentioned that he is removing “hl7” in the attribute identifiers. For example, in Table 2: urn:oasis:names:tc:xspa:1.0:resource: hl7:type would become: urn:oasis:names:tc:xspa:2.0:resource:type 6. Outreach The TC believes some outreach activities are in order, especially to government agencies that desire to use the NwHIN, such as the Center for Medicare & Medicaid Services (CMS), Social Security Administration (SSA), and the Substance Abuse and Mental Health Services Administration (SAMHSA). Mike Davis outlined some of the agency-specific requirements, e.g. in the SSA use case where authorization must be pushed to the organization from which a request for patient information is made. David pointed out many are assuming on the NwHIN that if the request is allowed from the information holder side, the legality of the request from the requester side is not considered. David offered to call CMS contacts he met at HIMSS 2012. Since the hour was almost over, David proposed that additional review of the SAML profile be done in the next meeting, April 27. Duane offered to clean up the document and sent it out prior to the next meeting. The committee decided that the profile should be put into final state then sent to the SSTC, since they were the TC that passed the original standard. 7. Action Items AI001: Duane will look into contacting NwHIN Specification Factory. AI002: Duane will look into how changes will affect current NwHIN implementations.
