Context/value Association (CVA) specification version 0.6 draft 1

Fellow CLRTC members,

I have finally created a revision of CVA suitable for review by 
committee members.  This is a significant revision with many changes 
that incorporate:

  - editing of prose based on feedback from Tony
  - addition of value tests (in addition to existing value lists)
  - addition of instance-level metadata (using UN/CEFACT supplementary
    components for an example)
  - repair of some inadvertent optionality in some attributes
  - change of namespace conventions to match genericode conventions
  - embedding of conformance criteria in the schema annotations
