This draft finishes descriptions of new elements, and addresses numerous
editorial comments, primarily those on this page (and two related pages -
links near the bottom):

Reviewers are free to post additional review comments on that page. See the
"Additional comments" section near the bottom.

These cleanup items still remain:
1. Re-organization of the contents - mostly re-grouping or alphabetizing
the bookmap and xNAL sections
1. Many elements, particularly new bookmap elements, are missing an example
section.
