indexterm syntax

I'm currently using
<indexterm xml:id="svrl">
   <primary>svrl </primary>
</indexterm>

in a book.

The generated index (html output) links back to the chapter
start, rather than to the indexterm.

Using v5 and the provisional stylesheets.
Is this not ready yet please?

Note that there is nothing in TDG to indicate
where the target of a link will be?
