some final polishing of WD40 (the first 50% up to line 1088)

Hi Stefan, Nick, JC, Ed and all

Some point Most of the things are for a final cleanup, what however 
concerns me is that some result codes have gone missing since WD37.

I hope that's it and we are done.

P.S.: Ed and JC: please look at lines 1050ff  for "shall" SHALL and 
"should" SHOULD.

* Lines 337 - 338 should probably be under <InlineXML> (i.e move 15 
lines further down).
* Line 357: shouldn't we have a reference for MimeType.
* Lines 358 and 359 could be suppressed.
* All schema fragments, Eg. Lines 376 - 395, 800 - 806: harmonize the 
indention (always use 2 or 4 spaces, don't mix) and synchronize with the 
schema file.
* Section 2.6: I think some result codes have gone missing.
* Lines 637 - 639 refers to result minor NotSupported however this one 
has gone missing (well have to define 
urn:oasis:names:tc:dss:1.0:resultminor:NotSupported in 2.6 or there).
* Remove several empty lines e.g 136, 276, 322, 360, 418, 497, 564, 569, 
588, 599, 603, 606, 613, 617, 620, 628, 723, 747, 770, 777, 780, 782, 
898, 914, 917, 931, 1069, 1074  etc ... Use line spacing where extra 
space between lines is necessary.
* Line 852 The reference should be "section 2.1 Well-Formed XML 
Documents [XML 1.0 (Second Edition)]" 
(http://www.w3.org/TR/REC-xml/#sec-well-formed) instead of [Schema1].
* Line 872: correct the alignment: I think this can be done by pressing 
ALT+Enter, Shift+Enter or Tab after the dot.
* Line 875: What Requester Error? --> again :

or to be defined there?
* Line 930: again :
