Question about citations / references / identifiers

Like Grant, I am now confused about identifiers and where they fit.

In our definitions of the terms we're using, does a 'reference' contain an 'identifier'? Or does a resolver take a reference and use it to find or construct an identifier that it then uses to navigate to the document? 

Perhaps laying out the relationships of these pieces will help us answer some of these open questions.
