[Issue 2] Inlined proposal v2

I didn't get any feedback on section 6 changes of inlined proposal v1 
[1], so I'm assuming that folks are ok with it.

Attached is inlined v2, that includes all the missing references in 
section 1.2 and changes to ensure that references to section 5 now point 
to section 6 instead. I also added four introductory sentences.

PS: I got a copy of Word 2007 and used it to generate this proposal 
(previously I was using Word 2k). I would be interested in finding out 
if other versions of Word/OOo have any problems viewing the document.
