I suspected this was an issue, so I business cards could be at least attached (linked or embedded) using the document reference.

I agree with this.

The other possibility is to directly structure the BusinessCard to allow a collection of business cards. 

Something like:

BusinessCard              (A message to exchange one or more business cards)
- BusinessCardInstance (0..n)
  ... all card data is here ...

Naming is not very nice then... better your proposal.
