[bt-spec] proposed solutions to issues

I've put proposed solutions in for the issues that Tony listed in the notes
from Wednesday's meeting as being ready for email vote - changing some of
the text a bit in some cases to align with the pattern of the rest of the
document and/or list discussion.  Some of the issues in Tony's list have
already been closed, usually accepting his proposal, I think.

I've also put in proposed solutions for a couple of others.

There are now proposed solutions for 15,16,19,39,50,60,67,95,102.


I'll make a 0.9.2.1 with these in sometime.
