Thanks John - I attached a screen shot to make sure my mean was clear.
I just downloaded the spread sheet and which I clicked on the icon
circled in the image, the form (behind it in the picture) opened. It
should also show the URL when you hover over it.

If that did not work for you then I will have to figure out why.
