To close on the indexing issues, shouldn't we come to a resolution on:
- Determining range from containment
- Optional global definition of synonyms and sorts
- See as a synonym without instances
- Indexing based on <keyword> or <term>
(Eliot's "mentions" -- possibly related to issue 43)
The revised proposals don't seem to resolve these issues.

Document Description:
The current indexterm element cannot express the
full range of indexing
semantics needed for production book indexes. The
inability to express
"see" and "see also" index
entries is one glaring example. This proposal
introduces new child elements of indexterm that
address this missing
functionality.
