Proposal 13111 (MathML) Updates 26 May 2013

I have posted updates to the MathML proposal (13111) to OASIS. The changes
include:

1. Added <mathmlref> element to the MathML domain

2. Added the equation domain to the proposal

3. Provided an HTML implementation for both domains as a Toolkit plugin,
including support for <mathmlref> when using @href (I'm not sure how to
implement @keyref resolution in the current Toolkit's HTML processing or if
it's even necessary.

I've been testing using the RNG-based files because RNG is now my master
format for vocabulary--the DTD declarations should be right but the sample
shell DTD is not (I accidently hosed it when testing our RNG-to-DTD testing
code) and I ran out of time to fix it before Tuesday.

I removed the XSD declarations entirely because they will be generated later
and I'm not going to waste time manually creating them.

This should be enough for Robert's team at IBM to start evaluating the
design.
