Issue 33: Replace @copy-to Stage 2 Updated

I've updated the Issue 33 Stage 2 proposal to reflect feedback from the last review. The DITA source is committed to SVN and the PDF is in Cavi.

The main changes I made were:

- Changed the resource-type value "deliverable-uri" to "deliverable-anchor" to emphasize that the value *contributes* to the deliverable URI but does not *specify* it.
- Clarified the intent for values of @appid when the resource type is deliverable anchor
- Refined the SHOULD and MUST statements
- Reordered the examples so the resourceid example is first.
