I can confirm that the navheader is missing from the output
html for the test/book.xml file being processed with
html/chunk.xsl in the XSL stylesheets version 1.23. I also
tested it with XT and Saxon.
The problem is in the chunk.xsl template named
header.navigation. Change:
<xsl:if test="suppress.navigation != '1'">
to
<xsl:if test="$suppress.navigation != '1'">
