I don't know what is causing the problem in pdfxmltex. I think it may have to do with it not ignoring the 'space-before' property on the paragraph content at the top of a list-item-body (all the other processors do). It may be necessary to customize the stylesheet so that the first para in a listitem has a zero space-before value.
