Problem b) is that you want any rollover lines in the toc to be indented, right? You could use a fo:list-block for each line, putting the section number in the list-item-label and the title, leader, and page reference in the list-item-body. Then any rollover lines will line up under the title. The solution to problem a) might be to experiment with the values of end-indent and last-line-end-indent.
