Actually, the problem is not the DocBook XSL stylesheets, but the current version of FOP, which is what I presume you are using.  FOP does not implement the line position adjustment properties, so there is no way to change the position in FOP.  If you are using XEP or XSL Formatter, then you will find that by default the bottom of the image aligns with the baseline, which is better.  And those processors respond to the line adjusting FO properties, AFAIK.  Can you change FO processor?
