You could imitate the templates in autotoc.xsl that generate a table of contents (there is an autotoc.xsl in both html and fo directories). The template named 'division.toc' generates the book-level TOC. Copy it to a customization layer and start changing the select patterns to add your element names. Then create a template with mode="toc" to match each new element, again imitating templates in that mode for elements that are similar.
