This seems to be a bug in FOP 0.93 regarding fo:footnotes inside a fo:list-block. If you set the stylesheet parameter glossary.as.blocks="1" then the footnote comes out ok. But it isn't as nice looking. I think you could report this as a bug to the FOP developers.
