Argh. Yes, I was mistakenly using templates from our package, even with
my stripped-down example (which I stripped down exactly to avoid this
kind of contamination).

Building cleanly gives me the same output as you. So the problem is in
our customizations, not in docbook. Thank you for the extra jolt of
sanity that I obviously needed, and sorry for wasting your time on a bug
in our code.
