Just so that I understand properly, if I wanted to associate an annotation
with, say, an image, wouldn't I write:

  <annot alt="the annotation
text"><mediaobject>....stuff...</mediaobject></annot>

And get an image that had the annotation text "the annotation text" associated.

Or, if the annotation itself was the image:

  <annot id="888">text words here</annot>

  <annotdef id="888"><mediaobject>....stuff...</mediaobject></annotdef>
