My mistake.  As you suspected, the document identifiers for LeadDocument and ConnectedDocument should have matched the identifiers in ReviewedLeadDocument and ReviewedConnectedDocument.  I believe we are in agreement.  I’ll go ahead and implement these changes in the next Working Draft.
