there was also another similar issue OFFICE-2398.
the resolution  there should answer your questions (4) and (5).

given that such puzzlement seems to be a recurring theme, how about adding something like this to the meta-field description:

"The exact way in which the content of the text:meta-field is generated from RDF metadata is unspecified."

re (0):
i agree that "generated upon" sounds weird.
perhaps "generated from" would be better? 
or "generated based on"?

re (1) (2):
the content should be generated by the producer that inserted the field. (and yes, this field can have mixed content, in contrast with all other fields)

re (3):
the consumer may update the content if it knows how to do that, otherwise leave the content as it is.
