PR to create WD04 of MQTT Transfer Specification

A new PR has been posted to update the MQTT Transfer Specification to WD04.

Highlights:

The operating model questions in 2.0 have been grouped into those with and without proposed answers. One new question, with proposed answer, has been added. Some new answers are provided and other answers have been refined.
Additional material has been added regarding message formats for the PUBLISH packet payload
Appendix A has been significantly revised.
Additional material has been added regarding handling of the Clean Session flag, along with an explanatory flowchart in new Appendix B.

I proposed to make a motion for a CSD ballot on this version at next week’s TC meeting.
