Test impacts of Issue 221.

As discussed on yesterday's call, the application
of the resolution of Issue 221 added three new normative statements to
the spec (ASM40012, ASM50045 and ASM50046).  I have just looked at
the CD02 version of the test assertions and test cases documents and there
are no test assertions and thus no test cases for these new statements.
