Simon,
I'm using Word 2007 so some details might vary.

If you have Acrobat (not Reader) installed, you should be able to
select "File/Save As..." and specify something.pdf as the file name.
There might be a place to specify file type as well.

When you have generated a PDF using this approach, open it with Adobe
Reader.  You will see a "Bookmarks" icon to the left of the document.
If you press this, the left sidebar TOC will appear.
