The recorded MP3 is a brilliant idea!  I enjoyed reviewing this and your PPT slides. 
 
Kudos for providing this - and mea culpa for missing the call.
 
Providing localizations of XSD schema for in-country usage - this is one area where CAM templates excel and make this quick and easy!
 
For example I'm already doing this for OASIS EML election formats. 
 
Simply ingest the XSD schema; select, exclude and extend as needed - then generate new subset XSD schema, CAM templates, xhtml documentation and XML test case instances.
 
Plus - aligning, standardizing and making interoperable across vocabularies is possible using an <Extensions> section in the CAM template to crosswalk between the specific usage and the alignment target.
 
You can also refine and restrict the content model in the CAM template - and use XPath expressions to control how and when that occurs.

With the <Extensions> approach we can look at a blend of SETesque approaches.  Could be very simple XML entries using familiar XPath target="" association="" approach - that CAM already uses in the main body of the CAM template - or one could blend that with more semantic style markup.
 
The clinical interoperable schema approach sounds VERY similar to the NIEM.gov approach - which the latest CAM template toolset has been designed to facilitate.

This is all complicated to do manually - but using template approach this is much more straightforward.  We have a whole library of XSLT to works between XSD excerpts and comparing to XML representation of vocabulary (such as NIEM).
 
Perhaps we can mock up something from some of the HL7 XML work you have already done?
 
Would you be able to provide a specific example for us?

Many thanks for bringing up the data types issue during yesterday's telco and commenting on that it would be good if HL7 data types are harmonized with UN/CEFACT CCTS datatypes.

Allow me to mention that there is an ISO effort "Health Informatics - Harmonized data types for information interchange (ISO/DIS 21090:2008)" in which ISO, CEN and HL7 are collaborating to harmonize the data types:
