Following on from yesterday's conf call discussion, 
attached is the proposed new layout for the Issues List.
The cover page inlcudes some proposals about how Issues 
are submitted and handled; the second sheet will be used 
for entering issues entered against the next version of 
the document, v8; and the last sheet contains the closed 
issues: on the last, I have *not* copied in the finalised 
information that Frank captued at the Vancouver F2F....
