You "sniped" away the important part of my posting:

I am aware of the solutions you described in your reply
but I was looking for the - nailed down - technical 
solution (both in the processing model and in the syntax, 
means DTD). This is completely missing in the specs - at 
least I have not found something - but it has to be there
to answer these very important issues.
