Just to note that I too had already booked at the Island Pacific
I found that they were OK about my booking by e-mail without
the proper form. I phoned and sent the e-mail to the contact on
the form, Suzanie Chan, and that was fine:
suzaniechan@islandpacifichotel.com.hk
